The heart automatically makes up for one lost heart, thus healing you. Killed players will drop a heart which then can be collected by either side. If you are not damaged the heart will not be used automatically, but will remain to lie where it is. You can manually pick up a heart lying on the ground by pressing the drop and pickup button(default: C). A carried heart will be located behind your back but will still heal whoever touches it automatically.
Note that the carried heart will not heal yourself automatically once you get hit, but only if you manually release it.
